Names of each known family member (including siblings, parents, spouse(s) and children. Include with this any known birth, marriage and death dates, and known places of residence.
Include the resources you have already searched. If you have already obtained documents, such as birth certificates, death certificates, etc, please let us know. Depending on your request we may ask for photocopies of those documents.
Include any other details that might be helpful such as what church they may have belonged to or what religion a family member might practice. Where they in the military? Are there any other family stories that were passed down?
Are there any other family surnames in the area that if information is found on them, that you would be interested in me recording?
Approximate time period or if an obit date of death.
